The exact legal name as filed with your state (for LLCs, corporations, partnerships)

Responsible Party Information

Individual who controls, manages, or directs the entity

✅ What You Need:

  • • Full legal name (first, middle, last)
  • • Social Security Number (SSN) or Individual Taxpayer ID (ITIN)
  • • Must be an individual person (not the business entity)

⚠️ Common Mistake: Cannot list the LLC or Corp as responsible party - must be a person

Physical location of your business (PO Boxes not accepted for principal address)

💡 Tip: Mailing address can be different (can use PO Box), but principal address must be physical

💡 Tip: Be specific but concise (10-15 words). Vague descriptions can delay processing.

Current and anticipated employment numbers

  • • Whether employees are household or agricultural
  • • Expected first wage payment date (if applicable)

✓ If Zero Employees: That's fine! Just indicate "0" - many businesses get EINs without employees.
